Description of Services

Community and Group Homes:  Medicaid supported homes which serve between six and eight people with developmental disabilities.  The emphasis is upon active treatment designed to help the individual become more independent.  These services are provided in a normal home environment.  People in these homes receive training in activities of daily living to prepare them for independent living or participation in a less restrictive environment.

Supported Independent Living:  Provides support for people living in their own apartment or home.  Support is offered according to what needs each individual has, allowing them to communicate the exact support they would like or require. Supports may fall into various categories including: money management, cooking, health and safety and socialization to name just a few. Staffing tends to be very flexible so each individual's needs can be addressed. Due to the flexibility of hours and emphasis on individualism, SIL has proven to be a successful and necessary component of community-based services.

Respite Care Services: Temporary care and supervision of children and/or adults who live at home with their families.  It is the provision of a predetermined number of hours/days of support whereby the principle caregiver is afforded the opportunity for rest and relaxation.  The most usual situation is for Evergreen to send staff into the individual’s home.

Family Support: Provides services that are geared for either children or adults with developmental disabilities who live with their families, and involves a system of supports based on the families and/or individuals needs and adaptive skills. 

Personal Assistance:  The provision of direct assistance with activities of daily living (bathing, dressing, personal hygiene, eating, meal preparation excluding cost of food), household chores essential to the health and safety of the enrollee, budget management, attending appointments, and interpersonal and social skills building to enable the enrollee to live in a home and community.

Supported Employment:  Supported Employment arrangements involve individuals working in the community.  Individuals either work in their own jobs, initially supported by a job coach, or in groups who go out into the community.

Vocational and Pre-Vocational Training:  Provides opportunities for people to develop work skills and functional life skills.  Day services provide a place outside the home for individuals with all types of disabilities to be active, socialize with their peers and receive needed health and personal care services.  Additionally, participation in a day program allows time for caregivers to work outside the home, attend to their own needs or the needs of other family members, or travel on a long- or short-term basis.  Vocational and pre-vocational training often occurs at the day program.

Children’s Choice Waiver: Provides supplemental support to children with developmental disabilities who currently live at home with their families, or who will leave an institution to return home.

Elderly and Disabled Waiver: Provides support to people to continue living successfully in their own homes. This service gives families another option of living at home instead of nursing home placement.

Case Management: Provides oversight and coordination of needed services for persons living at home.

Community and In-Home Waiver:  Provide support to people who live at home with their families.  The aim of the program is to help people acquire greater independence in their homes and participation in their communities.

Agency Companion: A person lives with a family or roommate who provides support to the person to be a part of a family unit.

Community Participation: A service that facilitates participation in community activities and encourages utilization of community resources.  This can also include participation in retirement activities and services that provide skill acquisition training.
